An exciting secondment opportunity has arisen for a full time Mental Health Practitioner fixed for 12 months to join our growing team at the Healthy Ageing and accredited Specialist Memory Service (SMS) in Milton Keynes.

An exciting secondment opportunity has arisen for a full time Mental Health Practitioner fixed for 12 months to join our growing team at the Healthy Ageing and accredited Specialist Memory Service (SMS) in Milton Keynes.
- The main responsibilities of this post will be to work within the Treatment Team pathway of the service.
- The post holder will offer support, advice and treatment to those presenting with the psychological and behavioural symptoms of dementia and patient’s 70+ diagnosed with other mental health conditions.

Main duties of the job
- To provide assessment of need, provide clear evidence which fairly and accurately determines the level of need and outcome identified by the service user.
- To hold a clinical caseload for families requiring short- and long-term support.
- To communicate in a skilled and sensitive manner, information concerning the assessment, formulation and treatment plans of service-users under their care and to monitor and evaluate progress during the course of their MDT care.
- To be responsible for the planning of Sec 117 meetings ensuring reviews are carried out within specific time frames.
- To work in a variety of settings e.g. service users’ home or Care Home.
- To administer long-acting anti-psychotic injections to patients as per prescription
- To liaise with other professionals, and work jointly with those involved with individuals and provide updates as appropriate.
- To work in partnership with other statutory agencies as well as the private and voluntary sector, care agencies, carers and advocacy groups.

- Registered with appropriate body e.g. HCPC/ NMC.
- Evidence of continuous professional development, e.g. practice teaching, mentoring
Essential criteria
- Post qualification experience of direct work with people with mental ill health difficulties.
- Experience of working with older people.
- Experience of working with people living with dementia/ carers of people living with dementia.
Essential criteria
- Knowledge of the theory and practice of specialised psychological assessment and intervention for people with dementia
- Knowledge of legislation and NICE guidance in relation to the client group and mental health.
