Job Purpose

Royal Surrey NHS Foundation Trust delivers acute and urgent care services within the community, supporting patients who may otherwise require hospital admission. The service provides rapid assessment, diagnostics, and treatment for individuals with complex health and social care needs, frailty, and exacerbations of long-term conditions.

As part of this team, you will undertake assessments in patients' own homes, residential care homes, and nursing homes to help prevent further deterioration in their health and wellbeing. Early assessment, detection, and intervention are key to reducing avoidable hospital admissions and supporting patients to remain safely within their community setting.

The service aims to provide urgent case management, crisis response, recovery support, and self-management strategies, while adopting a collaborative and patient-centred approach to care.

The service operates 7 days a week, from 8:00am to 8:00pm .

Job Summary

You will be part of a fast-paced and continually evolving Community Multidisciplinary Team (MDT), where your ability to make sound clinical decisions will play a vital role in supporting patient recovery and preventing unnecessary hospital admissions. You will work closely with patients' GPs and other healthcare professionals across primary and secondary care to deliver coordinated, patient-centred care.

As a Band 5 Nurse, you will be expected to undertake comprehensive risk assessments and make informed clinical decisions that have a significant impact on patient outcomes. You will work autonomously within the community setting while seeking support and guidance from colleagues and specialist services when required.

You will be competent in carrying out risk assessments and delivering clinical interventions in accordance with evidence-based practice and current clinical guidelines. Using specialist clinical skills, you will assess, plan, implement, and evaluate care for patients with a range of complex and varying healthcare needs.

This includes undertaking clinical assessments, urgent catheter care and management, wound care, medication administration, long-term condition management, and supporting patients to remain safely within their own homes wherever possible. You will also contribute to crisis prevention, recovery planning, health promotion, and patient self-management, ensuring high standards of safe and effective care are maintained at all times.

Administration of palliative stat doses

Insulin management and administration

Male catheterisation

Suprapubic (SP) catheterisation

Wound care and dressings

Digital Rectal Examination (DRE)

Palliative care support and assessments

Medication management and administration (including subcutaneous and intramuscular injections)
