Why this role would be great for you
Nightingale House is a resettlement service that provides 25 beds of accommodation to adults that have recently been discharged from hospital following a mental health admission. We work in partnership with Merseycare and our service helps people to build life skills, settle back into the community and find suitable long-term accommodation. The post holder will hold a caseload of service users at Nightingale House & Whitman House – which is a step-down service and move on option from Nightingale. As a Support Practitioner at You Matter you will be an empathetic and caring individual dedicated to providing unconditional support to those experiencing homelessness whilst supporting their mental health recovery. You will be part of a friendly team to deliver an effective, responsive, and resident-focused housing management service. You will play a key role in creating a safe, supportive environment while supporting residents to understand their goals and successfully transition into longer-term accommodation.
What your week may look like
- Welcoming new residents in line with the service’s policies and procedures.
- Developing risk assessments and support plans alongside people to continue their journey towards independence.
- Ensuring the basic needs of residents are met including good quality accommodation and facilities, healthy food, advice and guidance.
- Responsible for promoting and safeguarding the welfare of adults/children/young people who are at risk.
- Advocate for social inclusion of the people you support ensuring they can access health and financial support.
- Ensuring Housing Benefit and Services Charges are collected and managed effectively.
- Work alongside the people in our services to address their physical and mental health issues, including liaison with primary and secondary health teams.
- Carrying out the immediate support needs of residents as appropriate, providing crisis intervention where necessary and working with services.
- Ensuring all health and safety and maintenance issues are raised in a timely manner.
- Participating in team meetings, supervision and reflective practice encouraging open, honest, and transparent feedback where continuous improvement is encouraged.
- Being flexible and responsive to the changing needs of the service, ensuring our SHREK values run through all the work that you do.
- Promoting a supportive and inclusive culture within the service, valuing people and their diversity.

The skills we are looking for
- Experience of supporting individuals with complex needs, including mental health, substance misuse and housing instability
- Demonstrable leadership or supervisory experience
- Experience of multi-agency and partnership working (e.g. Mental Health, Social Care) • Experience of risk assessment and safety planning
- Experience managing incidents, maintenance or health and safety concerns
- Experience leading or supporting a staff team in a high-risk environment
- Strong administrative and case-recording skills
What is a nice to have
- Qualification in Health and Social Care, Mental Health or another relevant subject
- Experience working in a customer facing role and a desire to support people to make a positive impact/change in their lives
- Experience of working within a psychologically informed working environment
- Knowledge of housing management and IT databases
- Awareness of benefits and how they work including Universal Credit and other welfare systems
