Our Supported Discharge Unit (Ward 6 & 9) within Victoria Hospital are looking to add trained staff to their team.

Within this role, the successful candidate must be registered with the Nursing and Midwifery Council. Additional skills such as venapuncture, cannulation and IV additive experience is preferred but not essential. Staff will be supported to achieve additional clinical skills including catheterisation and syringe drivers.

The Supported Discharge Unit is a fast paced environment where we support our patients to achieve a safe a successful discharge. Within this role, we work closely with our MDT colleagues including Physiotherapy and Occupational health to ensure our patients are safe. We also work with the Health and Social Care Partnership and Social Work to ensure that patients journeys and discharge are safe and meet the needs of our patients.

Supported Discharge Unit care for patients with complex health and social care needs including End of Life Care Patients.

The patient discharge journey should start from admission and the Supported Discharge Unit play a key role in supporting hospital capacity through timely and safe discharges.

It is essential that candidates can demonstrate strong communication skills.
