About the Role:
You will be an experienced, registered Social Worker, with considerable post qualifying experience and have a range of social care knowledge, including experience of community working and safeguarding.
You will hold a varied caseload across user groups such as people with physical disabilities, learning disability and neurodiversity and mental health. Transferable skills can be considered.
The team collaborates closely with other teams engaged in the provision of Adult Social Care. Our main objective is to promptly assess current care and support, employing a strengths-based approach to work with individuals in order to enhance wellbeing, uphold rights, foster independence, and facilitate positive outcomes.
You will work closely with providers to ensure people’s care and support plans reflect their eligible needs and up to date costings and contribute to the team performance and positive outcomes for our residents.
In addition we work closely with health colleagues to consider NHS Continuing Healthcare alongside our Care Act duties.
You will work within the legal framework provided by the statute, guidance, policies and procedures.

- Registration with Social Work England
