West Berkshire Council is seeking a dedicated and motivated Social Care Practitioner to join our Adult Social Care team. This is an excellent opportunity to work within a supportive and collaborative environment, providing high-quality social care to the local community.

About The Role

As a Social Care Practitioner, you will be responsible for:

  • Working directly with individuals to assess their social care needs and provide appropriate support and interventions.
  • Conducting assessments and reviews of care packages, ensuring that individuals receive the necessary support to maintain their independence.
  • Collaborating with other professionals and agencies to ensure holistic care delivery.
  • Providing advice, guidance, and support to individuals and their families.
  • Maintaining accurate and up-to-date records of assessments, interventions, and outcomes.

About You

The ideal candidate will possess the following skills and attributes:

  • A relevant social care qualification and registration with the appropriate professional body.
  • Experience working in a social care setting, ideally within an adult social care environment.
  • Sound knowledge of relevant legislation, policies, and best practices in social care.
  • Excellent assessment and intervention skills, with the ability to work collaboratively with individuals and other professionals.
  • Strong communication and interpersonal skills, with the ability to build positive relationships with service users, colleagues, and external stakeholders.
  • Effective organisational and time management skills, with the ability to prioritise tasks and manage a diverse caseload.
  • A positive and solution-focused approach to problem-solving.
