Team: Adult Carers Team

Hours: Part time, 22.5 hours per week.

We currently have an opportunity for a Social Care Facilitator in our Adult Carers team.

Based at Moat Lane, central Solihull, this small and supportive team is responsible for supporting informal carers for adults in Solihull, often family members. You will work with carers to complete Carers Assessments and reviews, working creatively in partnership with carers to provide guidance and support. As specialist carers workers you will have a role supporting social care colleagues to develop best practice for all social care teams.

As a Social Care Facilitator in Solihull, you will work to a strengths-based model of assessment and support, promoting independence and well-being to achieve better outcomes for people, carers and their families.

To be successful in this role, you will be someone who can build a rapport quickly and able to prioritise and plan effectively and be creative with available resources.

To be successful in this role you will need to have the following:

  • Level 3 qualification in Health or Social Care (or equivalent).
  • Maths and English GSCE Grades A*- C (4-9) or equivalent (or be able to demonstrate an equivalent level of numeracy and literacy).
  • Experience of working within a social care environment and delivering person centred working, supporting people to take positive risks.
