BCHA are working in partnership with Dorset Healthcare, The Lantern Trust, Help and Care, Dorset Mind and the Harmony Centre, to deliver wellbeing hubs, across Dorset. This is part of the universal offer of wellbeing support, an element of the mental health transformation programme for adults. The hubs will support people to take the steps that matter to them, by connecting them to what they might need, at the right time and in the right place, ultimately empowering people to make their own choices and moving towards full citizenship within their communities. Citizenship in this context means having a voice, a sense of belonging, purpose, identity and equal access to the rights and opportunities that should be available to all.

Key Responsibilities:

Understanding of safeguarding and risk assessments

- Empower individuals by recognizing their strengths and supporting their self-management.
- Facilitate collaborative decision-making and advocate for individuals in multi-disciplinary meetings.
- Promote local resources and opportunities to enhance community well-being.
- Develop personalized learning and well-being plans and co-create safety plans with a focus on support rather than risk.
- Provide support through various communication methods, ensuring confidentiality.
- Address health inequalities and engage communities with effective strategies.
- Build relationships with local health, social care, and voluntary sector professionals to ensure appropriate referrals.
- Maintain accurate records and manage caseloads, promoting self-care and well-being.
- Encourage active citizenship and community engagement.
- Commit to ongoing reflective practice, supervision, and professional development.
- Support individuals experiencing significant emotional distress and maintain professional conduct.
