Healthcare Support Worker - Community Rehab - Band 3 - 37.5 Hours per Week - Fixed Term until 31.03.2025

North Staffordshire Combined Healthcare have a band 3 Healthcare Support worker vacancy within the Community Rehabilitation service based at Hilda Johnson House. This 24-hour community based service offers supported living for those moving forward with their recovery journey. The successful applicant will need to work to a rota, which will include some evenings, weekends and night shifts. Due to the nature of the service, lone working may be required if needed.

This is a full time 37.5 hours per week, fixed term to 31.03.2025 post that will involve providing support and therapeutic time alongside promoting independence to re-establish the person’s place in the community. The successful applicant will need to demonstrate enthusiasm, innovation with an appreciation and understanding of the concept of person centred recovery. Whilst experience of working in mental health would be desirable, excellent interpersonal skills will be key as full training will be given for the role. The team have an extended range of multi-disciplinary members including consultant, senior nurses, psychologist, Support Time and Recovery workers, social workers and Occupational Therapists.

The post holder will be given a full induction, regular supervision and personal development reviews with opportunities to work in other areas of the rehabilitation service to gain experience and development.

The post-holder is responsible for the provision of planned person centred care working in partnership with service-users.

The post-holder is responsible for working collaboratively with other Trust services to provide seamless care for service-users on defined care pathways.

The post-holder is responsible for working in partnership with agencies external to the Trust supporting service-users access to services in the independent and non-statutory sector.

To be part of the team providing 7 day 24hr cover to Hilda Johnson House low level rehabilitation facility, at times undertaking lone working to support service users residing at the unit and within the community.

(There may be occasions when the post holder is required to work across the rehab pathway and across other areas of the trust).

To provide support and spend therapeutic time with an allocated group of service users.

To promote and support the physical health, comfort, wellbeing and recovery of service-users, and to assist them in re-establishing their place in the community, as appropriate.

To assist Registered Professionals in assessments and care planning/ implementation and evaluation of individualized care plans.

To provide support and day-to-day supervision/induction staff members, demonstrating the Health Care Support Worker role and role modelling good practice
