Requiring the ability to travel efficiently across the area

- Individuals with long histories of rough sleeping or complex needs
- Young people aged 18–25 who are at risk of, or experiencing, homelessness
About The Role
As a Support Worker, you will play a vital role in empowering individuals to move forward from homelessness and rebuild their lives. You will provide tailored, holistic support to help customers achieve independence, stability, and improved wellbeing.
Key Responsibilities
- Provide dedicated support to individuals with complex needs or histories of rough sleeping, helping them recover from trauma and rebuild independence
- Manage a caseload of customers, identifying appropriate housing solutions and delivering personalised support interventions
- Develop and maintain support plans, risk assessments, and management plans using the INFORM system
- Work collaboratively with local agencies and partners to deliver a coordinated, holistic support service
- Support customers to maximise their income, including ensuring appropriate benefits are in place to sustain tenancies
- Assist with social housing applications, referrals to supported accommodation, and access to external services
- Advocate on behalf of customers and support them with paperwork, grant applications, and day-to-day needs
- Refer customers requiring social care assessments to the appropriate Social Services teams
