Programme Overview
The Community Ageing Well Programme is a new initiative developed in partnership with the Nottingham & Nottinghamshire Integrated Care Board. The programme aims to support older adults to remain independent, confident and socially connected by embedding healthy ageing education and strength & balance taster sessions within existing community groups across South Nottinghamshire.
The “Older and Wiser” guide forms the foundation of the educational offer, complemented by guest sessions from trusted partners including Nottinghamshire Fire & Rescue, Police Community Support Officers, NHS professionals, VCSE organisations, Adult Social Care and Social Prescribers.
Role Purpose
As the Community Ageing Well Practitioner, you will deliver engaging ageing-well sessions within established community groups, focusing on prevention, confidence-building and early support. You will build strong relationships, provide strength and balance taster sessions, coordinate partner involvement, and gather insight to support the programme’s evaluation and wider system learning.
This role is essential in promoting independence, reducing loneliness, and helping older people feel more in control of their wellbeing.
Duties and Responsibilities:
The postholder will deliver engaging healthy ageing education sessions using the Older and Wiser framework, alongside strength and balance taster sessions that promote independence and falls prevention. Sessions will be tailored to community needs, with guest partners involved where appropriate, and will focus on building positive, trusting relationships while representing Your Health Notts in the community.
The role includes collecting and reporting key data and insights, maintaining accurate records, and contributing to partnership reporting requirements. You will create an inclusive environment for older adults, using person-centred approaches to support behaviour change and provide signposting to relevant local services.
Working collaboratively with statutory and VCSE partners, you will strengthen local pathways, attend community events, ensure safe session delivery through risk assessment, and contribute to ongoing programme development and continuous improvement.

Skills and Competencies Required
Essential
- Experience delivering group education, community sessions or behaviour change programmes.
- Experience working with older adults or vulnerable communities.
- Excellent verbal, written and non-verbal communication skills.
- Strong organisational skills with the ability to manage a varied workload.
- Ability to build rapport and engage sensitively with diverse groups.
- Confident in data collection, capturing feedback and maintaining accurate records.
- IT literate (Word, Excel, Outlook and digital reporting tools).
- Ability to work independently and as part of a wider multi-agency team.
- Ability to travel across South Nottinghamshire.
- Enhanced DBS (or willingness to undertake).
- Flexibility to work occasional evenings/weekends.
