Summary
Accountable to: Cornwall Primary Care Training Hub
Hours: Via Staff Bank 1 day (7.5 hours) per week
Pay rate: £24/hour weekdays / £35/hour weekends
The role: CIOS ICB have funded a 6-month programme of Clinical Facilitation for Respiratory, Diabetes and CVD. There are up to 3 roles available within each long-term condition (9 roles in total) to provide support across the County with each facilitator working an average of 7.5 hours per week within a specific PCN/ICA related geographical area.
