Primary Care Advanced Clinical Practitioner - £54,075 to £60,638 dependent upon experience, yearly salary increments, NHS Pension
Working Hours: 36 hours per week, Monday to Friday
Location: Ashton GP Service, Old Street, Ashton-Under-Lyne, Greater Manchester, OL6 7SR

We have a fantastic opportunity for an Advanced Clinical Practitioner to work at Ashton GP Service based in Ashton Under Lyne.
As an Advanced Clinical Practitioner, you will work alongside a supportive multidisciplinary team who have a real commitment to continuous improvement, shared learning and best practice, while being passionate about providing patients with the highest possible quality care.
You will be supported on site by our Salaried GPs, Medical Lead, Nursing and Administration Teams, along with practice management and the wider gtd Team.

As an Advanced Clinical Practitioner, your duties and responsibilities will include:
- Using EMIS, you will consult via telephone, video or face to face, according to patient needs. (Full training and support for remote consultations will be available if required). You will be supported by the non-clinical team who have a good knowledge of our extensive local hospital and community services.
- The clinical team includes GP's, Practice Nurse, Pharmacist, Paramedic and HCA. You will possess leadership qualities and provide support to the wider team, act as a positive role model and proactively identify areas for quality improvement. You will proactively contribute to improving health outcomes for patients by identifying new initiatives / ways of working and as such promote health improvement and disease prevention.
- In addition, you will assist to further improve standards of care and, achieve QoF targets as well as KLOE objectives, as outlined by CQC.
- You will have full access to our gtd learning academy and clinical forums, including the NMP monthly forum, which is available to all our staff. This provides a great opportunity to grow and expand your clinician skills/knowledge and to excel as an ACP. You are also allocated CPD time weekly.
- The extensive footprint of the organisation provides you with an opportunity to be exposed to a wide variety of different services such as urgent care, specialist services and out of hours working, where shared learning is encouraged.
